Care Facilitator, Medical Assistant
Job Description:
- Respond to incoming patient and family questions and escalate emergent triage calls to the clinical team
- Proactively contact patients for longitudinal care management
- Coordinate and schedule appointments and referrals
- Facilitate communication and collaboration among healthcare providers, patients, and families
- Serve as a liaison between services and external providers
- Monitor patients' progress and response to treatment, identifying changes or complications
- Communicate escalations or new health risks to clinical leadership
- Maintain accurate communication logs and comply with documentation standards and regulatory requirements
- Participate in quality improvement initiatives and performance improvement projects
- Identify and implement strategies to improve care coordination, patient outcomes, and patient satisfaction
- Adhere to HIPAA regulations and maintain patient confidentiality
- Ensure compliance with clinical guidelines and standards of care
Requirements:
- Some College or associate degree in clinical sciences, Nursing, or a related field
- Minimum two (2) years of experience collecting referral information in the healthcare market
- Medical Assistant certification preferred, but not required
- Experience with electronic medical record software
- Proficient use of the Microsoft Office suite
- Ability to multitask, prioritize, and manage multiple projects simultaneously
- Excellent written and verbal communication skills
- Ability to work independently or in a team setting
