Functional Screener, Part-Time
Job Description:
- Schedule member appointments for annual or change-based functional eligibility re-determinations
- Optimize time and travel resources
- Use the State Department of Health Services screening tool to assess members' functional abilities, health, and required supports
- Collaborate with staff to align assessment data with internal records
- Conduct in-home, face-to-face interviews
- Enter assessment data into the designated online system to determine level of care
- Submit documentation promptly according to established guidelines
- Attend meetings and training on processes, screening tools, and state directives
- Participate in quality assurance activities
- Maintain required certification through ongoing skills testing
Requirements:
- Four-year bachelor's degree in a Health or Human Services related field with one (1) year of experience working with at least one of the Family Care target populations OR Wisconsin licensed Registered Nurse with one (1) year of experience working with at least one of the Family Care target populations
- Long Term Care Functional Screener (LTCFS) certification requirements; selected individual must complete online screener certification training course(s) and become certified as a functional screener
- Valid state driver's license
- Proof of personal vehicle liability insurance with at least 100/300/100 limits
- Ability to work part time Monday–Friday between 8:00 am–4:30 pm CST
- Ability to travel up to 50%
- Required TB screening for this patient-facing role
- Dedicated workspace protecting member PHI/HIPAA information
- Home internet with minimum 25 Mbps download and 10 Mbps upload speeds
