We partner with home health agencies to manage high-risk, medically complex adult and geriatric populations across Medicare, Medicare Advantage, and Medicaid. Operating as a virtual extension of the clinical team, we combine RNs, physicians, care navigators, and pharmacists supported by cutting-edge technology to provide continuous monitoring and rapid post-discharge follow-up.

As a Virtual RN Care Manager, your core focus will be managing a panel of 80–100 high-risk, post-discharge patients assigned from a single partner organization. You will create personalized care plans, perform proactive telephonic outreach, and step in during complex clinical escalations with one overarching mindset: "Whatever it takes to keep the patient safely at home."

What You’ll Do: Proactive Care Coordination & Clinical Monitoring

Panel & Care Plan Management: Oversee an assigned panel of 80–100 high-risk post-discharge patients (receiving ~3–4 new admissions weekly); conduct initial post-discharge assessments and build tailored, proactive care plans.
Proactive Outbound Outreach: Conduct routine telephonic follow-ups and symptom monitoring to close care gaps, address social determinants of health (SDOH), and prevent avoidable ER visits and hospital readmissions.
High-Acuity Re-Engagement: Re-engage with established patients to handle higher-level clinical escalations, including complex wound care issues, uncontrolled symptoms, medication management, or acute deterioration.
Interdisciplinary Escalation & Collaboration: Partner closely with physicians, APPs, Clinical Coordinators, pharmacists, and field home health staff to adjust care plans and escalate urgent medical concerns.
EHR & Analytics Optimization: Document all assessments, care coordination, and risk indicators in real time within Canvas EMR using modern analytics and clinical management tools.
Structured Virtual Collaboration: Actively participate in scheduled team touchpoints, including mandatory daily morning huddles (9:00 AM) and weekly virtual collaboration meetings.

Schedule & Location - EST - Eastern Standard Time - Nurses to Work New York Time!

Hours: Monday–Friday, 8:00 AM – 5:00 PM EST (Includes a 1-hour lunch break).
Mandatory Huddles: Daily at 9:00 AM; Tuesdays at 2:00 PM; Wednesdays & Thursdays at 8:30 AM.(All Virtual)
Setting: 100% Remote / Work From Home. Occasional periodic in-person meetings, client trainings, or quarterly team retreats may be required.
Equipment & Stipends: Laptop provided; includes a $40 monthly internet stipend and NY state nurse license renewal coverage.
Optional Triage Coverage: On-call triage participation is not required, but after-hours coverage opportunities are available for additional stipend compensation.

Qualifications

Education & Licensure

Registered Nurse (RN), currently licensed and active to practice in New York State.
Certified Case Manager (CCM) preferred.

Experience

Minimum of 5 years of RN experience with a strong background in Complex Medical Case Management and Palliative Care.
Experience managing Medicare, Medicare Advantage, or Medicaid populations.
Demonstrated familiarity with value-based care principles, HEDIS measures, transitional care, and goals-of-care conversations (including hospice/palliative care identification).

Skills & Attributes

Tech & AI Savvy: Highly comfortable navigating EMR platforms (Canvas), modern analytics tools, and AI-enabled clinical software.
Clinical Empathy & Communication: Exceptional listening and soft skills, cultural sensitivity, and an intimate understanding of New York patient populations.

Compensation & Benefits

Salary Range: $90,000 annually.
Pay Frequency: Twice monthly (Paid on the 15th and the last day of the month).
