The role

  • 1099, 30–40 hrs/week in business hours EST (you set your schedule)
  • $40–60 / hour: factors include experience, geography, and client load
  • Serve elderly adults with chronic conditions and their families: educate on navigating Medicare and the healthcare system, coordinate care across providers, advocate for what they need

You’d be a great fit if you:

  • Hold an LMSW, LCSW, or RN (clinical license a plus but not required)
  • Have experience in geriatric care management, hospice, palliative care, discharge planning, rehab, or hospital social work
  • Love fostering connection with older adults and their families
  • Are a creative problem solver who wants a job that looks different every day
  • Are tech-savvy and excited by quickly evolving environments
  • Remote - must be located in the United States
